Dryness, burning & painful sex: what GSM actually is
There's a name for the cluster most women describe in a lowered voice: genitourinary syndrome of menopause, or GSM. Vaginal dryness, burning or irritation, discomfort or pain with sex, decreased lubrication, urinary urgency, and recurrent urinary tract infections all belong to it — and they share one cause. The tissues of the vulva, vagina, urethra, and bladder are rich in estrogen receptors, and as estrogen declines those tissues become thinner, less elastic, less lubricated, and more fragile.
Two things make GSM different from hot flashes, and both matter. First, it does not improve on its own — unlike vasomotor symptoms, which often fade with time, GSM is progressive without treatment. Second, it is strikingly treatable, and yet most women never raise it and are never asked. Please raise it. It's a five-minute conversation that changes how people live.

How we treat it
Local (vaginal) estrogen is the mainstay: creams, tablets, or a ring that deliver estrogen directly to the tissue that needs it. Because absorption into the bloodstream is minimal at these doses, a progestogen isn't needed for lining protection with local estrogen alone — a point of confusion we're glad to clear up. It's a different conversation from systemic hormone therapy, with a different risk profile, and many women who aren't candidates for systemic therapy can still use it. We'll go through your history honestly.
Non-hormonal options stand on their own or work alongside: hyaluronic-acid vaginal inserts, quality long-lasting lubricants and moisturizers, and pelvic floor physical therapy where muscle guarding is part of the picture. Energy-based devices — the treatments below — address tissue quality from a second direction, and work best on tissue that's hormonally supported. That's why our conversation starts with the biology and only then reaches the device.
