For some patients, medical cannabis is a reasonable part of managing a qualifying condition — and Hawaii law provides a clear, physician-and-APRN-supervised path to it. At Wahine Health, certifications are evaluated by Ann Craddock, MSN, APRN, WHNP-C, FNP-C, our dual board-certified nurse practitioner, with the same unhurried, clinically grounded visits we bring to everything else. Who this is for

Hawaii residents age 21 and older with a valid state-issued ID, who have a qualifying condition under Hawaii law. Qualifying debilitating medical conditions include ALS, cancer, glaucoma, lupus, epilepsy, multiple sclerosis, rheumatoid arthritis, HIV/AIDS, and PTSD — as well as chronic or debilitating conditions (or their treatment) producing severe pain, severe nausea, cachexia, seizures, or severe and persistent muscle spasms. Chronic pain lasting longer than six months is one of the most common qualifiers. The full list is on the Hawaii Department of Health website.

How it works

Step 1 — Apply with the State first. Hawaii requires you to begin at the Medical Cannabis Registry website: create an account, complete your application, pay the State's registration fee ($38.50 for one year), and select Ann Craddock, APRN as your certifying provider.

Step 2 — We review and call you. Ann reviews your application by the next working day. Once it's verified, our office will call to schedule your visit at our Wailea office. If you haven't heard from us within three working days, please call.

Step 3 — Your evaluation. A one-on-one visit covering your history, your qualifying condition, and a personal review of the potential benefits and risks of medical cannabis for your health. Bring your Hawaii ID and any medical records or provider letters documenting your condition.

Step 4 — Certification and your card. If certified, Ann submits your approval through the State Registry, and the Department of Health issues your 329 card to you electronically through the registry.

Choosing a Caregiver? Hawaii allows a designated Caregiver to grow cannabis for a qualifying patient. If you include a Caregiver on your application, they must come to your appointment with a valid Hawaii ID to complete and sign their portion of the forms.

Transparent pricing

Hawaii law (Act 241) caps what any provider may charge for medical cannabis certification, and our fees sit exactly at that cap — with everything included: your evaluation, counseling, all certification paperwork, registry submission, and record-keeping. No add-on charges.

New certification (1 year): $109.95 — the $105 capped certification fee plus Hawaii General Excise Tax.
Renewal (1 year): $109.95 · Renewal (2 years, available from your first renewal): $219.90

Payment is collected when you book; $50 is a non-refundable evaluation deposit, and if a certification can't be issued after your evaluation, the remainder is refunded. The State's registry fee ($38.50/1-yr, $77/2-yr) is paid by you directly to the Department of Health — we never collect it. Certification is not covered by insurance.

Common questions

Is certification guaranteed? No — and be cautious of anyone who implies otherwise. Certification is a clinical decision made by your provider after a real evaluation, based on Hawaii law and your medical documentation.

Is this a prescription? No. Qualifying patients receive a certification, which allows the Department of Health to issue your 329 Medical Cannabis Registry card.

Can I do this by telehealth? Not at this time — certification visits are in-office at our Wailea location. Telehealth may become available for renewal visits in the future.

Do you certify visitors or patients under 21? No. Our practice certifies Hawaii residents age 21 and older with a valid state-issued ID.

How long until I get my card? The Department of Health issues cards electronically through the registry. Once your approval is submitted, status questions go to the Registry directly — our office isn't notified once your card is issued.
